Abstract

In response to COVID-19, health systems have had to delay care and shift from in person care to telehealth. Telehealth provides care to patients remotely using phone, video, or other devices that can help manage care. With this enhancement, the research team wants to learn how these changes affect care and health outcomes for older patients with multiple chronic health problems. The team will look at whether COVID-19 affects some patients, such as those with low incomes, more than others. Also, the team will see if working with a case manager reduces risks from COVID-19. A case manager is a nurse or social worker who supports the patients' care plan and coordinates care.
