Abstract
The COVID-19 pandemic has led to enormous stresses on health and social service systems in New York City, putting the health and well-being of vulnerable populations at great risk and potentially exacerbating known racial/ethnic disparities. In this setting, New York State Medicaid has made adaptations, such as the use of telehealth and telephonic visits by care managers through the Health Home program. The enhancement to this project aims to estimate the impact of Health Home enrollment compared to usual care among Medicaid-insured patients with diabetes and other chronic conditions on access to health care and social service; COVID-19 and non-COVID-19 clinical outcomes; and racial/ethnic disparities as a result of these outcomes, before and during the pandemic.
